Membership Cancellation Form<\/h1><\/div>

Thank you for letting Carbon Performance Salt Lake City be a part of your fitness journey. Please review the terms and alternative options outlined below prior to submitting your request.<\/p>\n<\/div>

Terms & Contract Policies<\/span><\/h2><\/div>

  • 30-Day Notice:<\/b> If you decide to cancel your gym membership, we require at least 30 days’ notice. We will apply any billing charges scheduled within that 30-day window.<\/span><\/li>\n
  • Early Termination Fee:<\/b> If you cancel your gym membership before your agreement terms end, we will apply a $300 cancellation fee.<\/span><\/li>\n
  • Fee Waivers:<\/b> If you are still within your agreement terms, we will waive the $300 cancellation fee if you provide verifiable documentation for:<\/span>\n

    • Qualifying relocation from a Carbon Performance location, as defined by your membership agreement terms<\/span><\/li>\n
    • Medical reasons preventing participation<\/span><\/li>\n
    • Military deployment<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/div>

      If you are working around a busy period, relocation, or have a temporary concern, you can put your membership on hold, switch gyms, or adjust your membership package instead of quitting:<\/span><\/p>\n

      • Account Freeze:<\/b> Temporarily pause monthly payments without ending your contract or losing your locked price.<\/span><\/li>\n
      • Gym Transfer:<\/b> If you are moving closer to another Carbon Performance location, you can transfer your membership and keep your existing agreement intact.<\/span><\/li>\n
      • Package Change:<\/b> Reach out to our Salt Lake City staff to switch to a plan level that suits your updated schedule and fitness needs.<\/span><\/li>\n<\/ul>\n<\/div>
